This tool will guide you through the BASIS Study Parent/Guardian Information to help you decide whether to take part in the study.

When you press START, the information will be presented in full screen mode to remove distractions. On larger screens, you can switch back to your normal view using the Exit button in the header.

Follow the simple step by step process, reading to the bottom of each page before you move on to the next.

When you have viewed all of the steps you can return to the main study website or CONSENT to take part in the study.
